This form is where you provide us with details that we need to begin the arrangements and your personal wishes.

This is a statutory form which, once completed, is sent to the medical referee to authorise the cremation to take place.

This page will allow you to pay for your chosen (Willow or Birch) funeral choice as well as any extra choices such as ashes caskets.

Here you can upload any necessary documents (such as registrars forms) or photographs/readings for a service with our Birch Option.
